SalvoCare Eastern

The Frankston and Rosebud Crisis Centres are entry points for individuals and families who are homeless or at risk of homelessness. These Centres provide assessment, information, advocacy and referral to a range of accommodation and housing services. They can help navigate services and link in with services such as family violence, drug and alcohol, health and mental health, legal, financial, employment, education and training.

Housing for the Aged Action Group (HAAG)

Statewide service for people aged 55 and over on a low income. Able to assist with long term housing arrangements. This is not a crisis service.

Aged Care Assessment Services (ACAS)

ACAS teams are multi-disciplinary, independent teams that assess the needs of older people and their carers. They assist in accessing Commonwealth-funded residential aged care, residential respite care, transition care and home care packages. Under 65’s require an eligibility assessment.

Peninsula Health has a range of primary and community health services catering to the varied and complex needs of older adults including Aged Persons Mental Health Team (APMHT) and the Aged Mental Health Inpatient Unit (1West).

Older Wiser Lifestyle (OWL)

The OWL program provides support, counselling and education for adults over 55. The program encourages better health and wellbeing, drug and alcohol education as well as managing life stage changes.

Home Care Packages (HCP)

Packages are assessed by the individual’s identified need and include in home support services and basic home maintenance, personal care, clinical services such as nursing and allied health, and care coordination and case management. The four levels of Home Care Packages vary from basic care needs to high-level care needs.
